Position Summary:

The Hospitalist Program Nurse Practitioner, under the supervision of the Medical Director of the Hospitalist Program, will provide care for patients admitted to The Hospitalist Program of St. Luke's as defined by the Missouri Nurse Practice Act and institutionally approved guidelines. The Nurse Practitioner will work closely as well with all multi-disciplinary clinical specialists to collaboratively design a patient care plan for optimal outcomes. The Nurse Practitioner will facilitate needed health care across service lines within effective and realistic time frames. The Nurse Practitioner, as a vital part of the Hospitalist Team, will be integral to enhanced health outcomes, increased patient satisfaction and reduced costs of care. The Nurse Practitioner is expected to perform duties and responsibilities in a manner consistent with our mission and values.

 

Education, Experience, & Licensing Requirements: 

Education: Nursing Degree, Completion of a Nurse Practitioner Master’s Program

Licensure: Current licensure as a Registered Nurse in the state of Missouri, certified by the American Nurses Association as an Adult or Family Nurse Practitioner (FNP Required)

More specific details include:

  •  Performs patient evaluations: Takes medical histories and performs physical examinations on new patients admitted from the E.D., direct admits and transfers from other hospitals. Provides appropriate annotations in the medical record for all such evaluations performed.
  • Performs initial evaluation of consults.
  • Responds to all calls related to inpatients on the Hospitalist service.
  •  Works closely with the E.D., Nursing, Respiratory Therapy, ICU staff, etc. to coordinate the care of the Hospitalist patients during the night.
  • Completes all computerized admission order entry for all patients admitted during his/her shift.
  • Orders diagnostic and therapeutic services: Orders radiographic, laboratory and other diagnostic tests and procedures for the purposes of diagnosis or evaluation of our Hospitalist Program patients.
  • Performs diagnostic and therapeutic procedures as appropriate. Communicates plan of care and results with all members of the health care team.
  • Educates patients and families regarding patient issues, and the diagnostic/therapeutic plan indicated based on assessment and input from multidisciplinary team.
  • Assists in the coordination of care for patients admitted to the Hospitalist service.
  • Serves as a liaison between the patients and other members of the health care team.
  • Requests and interprets laboratory and radiographic data for incorporation into the patient's assessment and progress evaluation.
  • Communicates with family members on patients admitted during his/her shift as well as those of other patients on the Hospitalist Services as needed.
  • Continually updates professional knowledge base through continuing education activities.
  • Performs other duties as assigned by the Medical Director of The Hospitalist Program. The assessment, care and treatment by the N.P. will be consistent with the specific age-related needs of the patient.
